The following are requirements for participation in a Caregiver Orientation class. Work referrals can not be issued until all required documentation has been processed and the Caregiver Orientation has been completed.
- C.N.A. certification by Georgia Department of Human Resources. C.N.A. certification is not required for companion assistance.
- Current C.P.R. and First Aid certification.
- Current drivers license AND U.S. passport or birth certificate.
- Social Security Card.
- Employment Authorization Card, if applicable.
- Auto Insurance Card. All caregivers must have access to a reliable vehicle.
- Five year work history, including company name, address and phone number.
- Two emergency contact numbers.Five references from previous employers and/or educational institutes.
- General Liability Malpractice Insurance. This insurance can be purchased at orientation through CM&F Group.
- Annual TB screening.
- Criminal background report issued within 3 months of application date.
- Contractor Supply Packet (may be purchased through Easy Living Services.
