The following are requirements for participation in a Caregiver Orientation class. Work referrals can not be issued until all required documentation has been processed and the Caregiver Orientation has been completed.
C.N.A. certification by Georgia Department of Human Resources. C.N.A. certification
is not required for companion assistance.
Current C.P.R. and First Aid certification.
Current drivers license AND U.S. passport or birth certificate.
Social Security Card.
Employment Authorization Card, if applicable.
Auto Insurance Card. All caregivers must have access to a reliable vehicle.
Five year work history, including company name, address and phone number.
Two emergency contact numbers.Five references from previous employers and/or educational institutes.
General Liability Malpractice Insurance. This insurance can be purchased at orientation through CM&F Group.
Annual TB screening.
Criminal background report issued within 3 months of application date.
